Patient Portals

What a portal is

A secure website or app from a clinic or health system, such as MyChart, where patients see visit notes, test results, appointments and bills, and message their care team.

Signing up

Most portals need an activation code from the clinic, or identity checks online. Help the client use their own email, a strong passphrase and two-step verification.

The big four tasks

Check upcoming appointments, read test results, request a refill, and send a message to the care team. Practice each one until the client can do it alone.

Reading results

Results often arrive before the clinician has reviewed them. Show where normal ranges appear, and remind clients that the care team will explain what results mean.

Proxy access

A parent, caregiver or adult child can get their own login to help manage someone else's portal, with permission. Rules differ by clinic and for teens, so ask the clinic for its proxy form.

Coach Lucy Tip · Guide, don't drive. Keep the device in your client's hands and let them tap every step.

Live It · Scenario

Rosa helps her father

Rosa manages appointments for her father, who speaks Spanish and does not use email. She has been logging in with his password.

Your navigation steps

  1. Explain why sharing his password is risky
  2. Ask the clinic about proxy access
  3. Help Rosa complete the proxy form
  4. Set the portal language to Spanish if offered
  5. Practice finding his next appointment

In the Qwest, the steps are shuffled. Put them in the best order to help the client and earn your LIVE It coins.

Teach It Forward

The fastest way to master a skill is to teach it. Your Share It challenge:

  1. Help one person find their next appointment in their portal.
  2. Use teach-back: ask them to show you the step in their own words.
  3. Post what you learned (no client names) in your group or the Share It Studio to earn SHARE It coins.
