Intake and Documentation

Start with the person

Ask what they want to be able to do, not what they do not know. "I want to see my grandkids on video" is a better starting point than a skills test.

The four-question intake

1) What do you want to do online? 2) What device and connection do you have? 3) What has gotten in the way? 4) What would make today a win?

Set one goal per visit

Pick one goal the client can reach today, such as "log in to the portal". Small wins build confidence and bring people back.

Intake and Documentation continued

Consent and privacy

Explain what you will write down and why. Record only what you need. Store records the way your organization requires.

Outcomes that count

Record what changed, not just that you met: device set up, internet enrolled, portal activated, telehealth visit completed, benefits application submitted, referral made. These roll up into the PHIT Score Card.

Coach Lucy Tip · Guide, don't drive. Keep the device in your client's hands and let them tap every step.

Live It · Scenario

Ms. Begay's first visit

Ms. Begay arrives with a new phone from her son. She says she is "no good with technology" and needs to see her daughter's school messages and her own clinic appointments.

Your navigation steps

  1. Ask the four intake questions
  2. Choose one goal for today
  3. Explain what you will record
  4. Complete the goal together
  5. Log the outcome and book a next step

In the Qwest, the steps are shuffled. Put them in the best order to help the client and earn your LIVE It coins.
