The problem

Care moved online. Not everyone came with it.

Appointments, results, refills, benefits renewals and video visits now start on a screen. Residents without a connection, a device or the confidence to use them fall behind, and it shows up in missed care.

Access

No home internet or no working device.

Skills

Can text, but can't set up a portal or join a video visit.

Trust

Afraid of scams, so they stay offline.

Most digital navigator trainings run 8 to 20 hours, include little or no health content and end with a completion certificate. Your residents' biggest digital needs are often health needs.

What you get

A full program, not just a course.

DHN Pro seats

24 hours in 12 modules, the 8-hour work-readiness strand, the Navigator Qwest, final exam and certificate.

FREE Basics for all

The 10 Things eBook and Category Qwest for every resident, free, with no sign-up.

Group Leader Guide

16- and 24-hour schedules with an opener, activity and discussion for every module.

Cohort dashboard

Minutes, pre/post scores, exam results, badges and client outcomes in one place.

Webinars & roundtables

Monthly webinars, quarterly roundtables and leaderboards to keep cohorts moving.

Annual report

A reach and engagement report you can hand to your board and funders.

What you can prove

Numbers your funders ask for.

Minute-level time

Every learner's time on task, to the minute, exportable for WIOA and funder reports.

Pre/post growth

A pre-check and post-check for every learner, plus a scored final exam (80% to pass).

Client outcomes

Device set up, home internet enrolled, portal activated, telehealth visit completed, benefits application submitted, referral closed.

PHIT Score Card by ZIP

Outcomes roll up by ZIP code so you can see where help is landing and where it is not.

WIOA-ready exports

Hours, completions and credentials in a file your case managers can use.

Work experience

Optional 180-hour internship log with supervisor sign-off for youth programs.

Coach Lucy Tip

Pick two or three ZIP codes to focus on before you launch. A tight target makes your first PHIT Score Card report tell a clear story.

Questions

Good to know

How is time on task measured?

The FFH Academy tracks each learner's time in the course to the minute. Program leads see it on the cohort dashboard and can export it for WIOA and funder reports.

What do residents pay?

Nothing for FREE Basics. DHN Pro seats for your navigators are covered by your license.

How are client outcomes kept private?

Navigators log outcome types, such as portal activated, not client names. Outcomes roll up to the PHIT Score Card by ZIP code. The course also teaches navigators never to keep client passwords.

Can our own staff lead the course?

Yes. Choose train-the-trainer. Leaders get the Group Leader Guide with 16- and 24-hour schedules and monthly leader webinars.

Is it on our state's Eligible Training Provider List?

Ask us. Listing is state by state, and we will tell you where things stand in yours.

Is it available in Spanish?

Spanish is coming. Let us know if your residents need it, and we will share timing.

How does it connect to careers?

DHN Pro stacks into CHW Pro and Home Care & Digital Caregiving. See the full ladder.
