Coaching and Social Support

Coaching is not telling

The client sets the goal. You help them break it into steps, find what gets in the way and celebrate progress. Ask, "What would you like to work on?"

Action plans

A good action plan says what, how much, when and how often: "Walk 10 minutes after dinner, Monday, Wednesday and Friday." Ask how sure they feel from 0 to 10. If it is below 7, make the step smaller.

Stages of change

People move from not thinking about change, to thinking, to preparing, to acting, to keeping it up. Slips are normal. Meet people where they are.

Coaching and Social Support continued

Social support

Support can be emotional (listening), informational (useful facts), practical (a ride, a food box) or feedback that helps someone see progress. Help clients build their own circle, not depend only on you.

Self-management, within scope

Help clients follow the care plan their care team made: write down questions, track readings, set reminders. Never change medicines or doses. Questions about the plan go to the care team.

Coach Lucy Tip · Connect, don't direct. The client leads, the care team makes the medical calls, and you build the bridge between them.

Mr. Silva wants to walk more

Mr. Silva, 66, has high blood pressure. His doctor suggested more activity. He says he used to love walking but "never finds the time."

Your CHW steps

  1. Ask what he would like to change and why it matters to him
  2. Help him make one small, specific action plan
  3. Ask how confident he is from 0 to 10, and adjust the plan
  4. Help him find a walking buddy or a group
  5. Check in next week and adjust the plan together

In the Qwest, the steps are shuffled. Put them in the best order to help the client and earn your LIVE It coins.
