Individual and Community Assessment

Individual assessment

Learn the person's strengths, needs and goals. Use a screening tool if your program has one, but keep it a conversation. The client chooses the priority.

Screening with respect

Explain why you ask, who will see the answers and that they can skip any question. Tools such as PRAPARE or the CMS Accountable Health Communities screening tool ask about housing, food, transportation, utilities and safety.

Social determinants of health

Healthy People 2030 groups them into five areas: economic stability, education access and quality, health care access and quality, neighborhood and built environment, and social and community context.

Individual and Community Assessment continued

Community assessment

Look at data such as county health rankings and the community health needs assessment that nonprofit hospitals publish every three years. Then add the voices of residents through surveys, listening sessions and walks.

Asset mapping

Map what is strong: people, groups, places and services. A good assessment is shared back with the community it describes.

Coach Lucy Tip · Connect, don't direct. The client leads, the care team makes the medical calls, and you build the bridge between them.

First visit with the Martinez family

The clinic asks you to visit the Martinez family. Mr. Martinez lost hours at work, and the kids' school says they seem hungry on Mondays.

Your CHW steps

  1. Explain the screening, why you ask it and that they can skip any question
  2. Ask the screening questions in plain words
  3. Ask about the family's strengths and who supports them
  4. Agree together on the top priority
  5. Make a plan with next steps and dates, and document it

In the Qwest, the steps are shuffled. Put them in the best order to help the client and earn your LIVE It coins.
