Nutrition, Hydration and Meal Support
Follow the diet order
The care plan may list a special diet, like low salt, diabetic, soft foods or thickened liquids. Follow it exactly, even if the client asks for something else. Tell your supervisor if the client keeps refusing the diet.
Safe meal help
Have the client sit upright for meals and for a while after, as the care plan says. Offer small bites and sips, let them chew and swallow fully, and never rush. Let them feed themselves as much as they can.
Swallowing trouble
Coughing or choking while eating, a wet or gurgly voice, food left in the mouth or pain when swallowing can be signs of a swallowing problem. Stop and report these signs. Thickened liquids must be made exactly as directed.
Nutrition, Hydration and Meal Support continued
Hydration
Older adults may not feel thirsty. Not drinking enough can cause dizziness, constipation, urinary infections and confusion. Offer fluids often unless the care plan limits them. Signs to report include dark urine, dry mouth and new confusion.
Intake and output
Some clients need what they eat and drink measured. Record fluids in milliliters or ounces as your agency teaches (1 ounce is about 30 mL). Record meals as a percentage eaten. Report big drops in eating or drinking.
Words Caregivers Use

Trouble swallowing.
When the body does not have enough water.
Measuring what a person drinks and eats and how much urine or other fluid leaves the body.
Lunch with Mr. Davis
Mr. Davis is on thickened liquids after a stroke. During lunch he coughs after each sip, and his voice sounds wet. He asks for a glass of regular water.
Your care steps
- Stop feeding and let him cough
- Keep him sitting upright
- Explain you can only give liquids the way the care plan says
- Report the coughing and wet voice to your supervisor right away
- Record what he ate and drank and what you reported
In the Qwest, the steps are shuffled. Put them in the safest order to earn your LIVE It coins.
