5 · The C3 framework
C3 = Community Health Worker Core Consensus Project — the national framework of 10 core roles (cultural connection, health education, care navigation, coaching & support, advocacy, capacity-building, direct service, needs assessment/SDOH, outreach, evaluation) and 11 core skills (communication, interpersonal, service coordination, capacity-building, advocacy, education, assessment, professional conduct, evaluation, knowledge base, and more). Most state CHW programs map to it — but there is no single national CHW exam, so this track is framed to C3 and supplements your state-approved program.
Where CHW fits: the CHW track runs alongside the clinical ladder (CNA → PCT → LPN/RN), not on it. It's the community-facing branch of Force for Health's mission — the trusted bridge that connects people to everything else on the ladder.
