“I have orthostatic hypotension — my blood pressure drops when I stand up, which makes me lightheaded or faint. I move slowly when changing positions, stay hydrated, and take medications carefully. I wanted you to know because if I fall, especially hit my head, please help me get checked — falls are how this condition causes harm.”

What OH Actually Is
Blood pressure drops when you stand. Brain doesn't get enough blood briefly โ dizziness or faint.
Common Causes
Meds (BP, prostate, Parkinson's), dehydration, autonomic conditions, deconditioning.
Spot the Red Flags
Fall with head injury ยท loss of consciousness ยท confusion after fall ยท chest pain.
Triggers
Hot showers, big meals, dehydration, standing fast, new BP-lowering meds.
Know Your Numbers
BP sitting + standing. Drop of 20 systolic or 10 diastolic = OH.
Med Review
Many cases are medication-induced. Review every new pill with your team.
Hydration + Salt
Often increased fluids and salt help โ unless you have heart failure.
Slow Position Changes
Sit a beat, ankle pumps, then stand. Hold something stable.
Make Every Visit Count
Bring sitting + standing BP log and a list of all meds + supplements.
Your Care Team
PCP + sometimes neurologist (autonomic) + cardiologist.
Telehealth
Med review by telehealth works well.
Insurance & Cost
Compression stockings often need a Rx for insurance coverage.
Compression & Binders
Stockings or abdominal binders help some patients.
Teach & Mentor
Falls are devastating in older adults. Share what you've learned.
Advocate
Push for medication review at every visit โ OH is often missable.
Research & Trials
Newer drugs (droxidopa, midodrine), autonomic testing advances.
