GERD is treatable when the lifestyle and medication mix is right. Becoming a Certified Prepared Patient means you can run the lifestyle stack, take the medicine right, screen for the red flags (swallowing, bleeding, weight loss), and review long-term PPI use annually.

A short readiness assessment — what do you know, where are you confident, where do you want to focus. The Program onboarding page captures this in two minutes and writes your baseline.
The full GERD course — your body, your medications, your action plan, and how to bring it all into your visits. This is the heart of the pathway.
Your personal GERD action plan, symptom log, med list, and the message templates you'll use to contact your care team. The Care Partner Tools Kit is already in your folder — your Patient Tools Kit follows the same structure.
Knowledge becomes competency through observed practice. The Skills Lab is where you demonstrate the thermometer, pulse oximeter, inhaler (if needed), the patient portal, and the action-plan readback to a credentialed Prepared Patient Educator.
A pretest now and a post-test after the course — same 10 condition-specific items with teaching prompts on wrong answers. Once you pass at 80% on the post-test, you are eligible for the Certified Prepared Patient — GERD credential.
A gamified family + community activation card. Each square is a small advocacy action — talking with one family member about your plan, getting a flu shot, scheduling your next visit, sharing a resource. Earn the full card and you become an Ambassador in your community.
The Provider Hub is the clinician version of everything you just learned, in their tone and on their terms. Share it with your team so when you walk in prepared, they know what 'prepared' means here. Optional but powerful.
Once steps 1 through 7 are complete, your credential is issued — a verifiable digital badge with your name, the condition (GERD), the date, and the issuing institution (The Force for Health® Network). It can be reported to your health system EHR via FHIR.
Your credential preview. Once every step above is complete, this becomes a verifiable digital badge — issued by The Force for Health® Network and reportable to your health system through FHIR.
