Prepared Provider · Welcome

Train your patients. Trust your patients. Stay in your practice.

Prepared Provider is a brief CNE/CME-aligned program for clinicians and care teams. The premise is simple: prepared patients and prepared care partners are practice assets — fewer 3 a.m. calls, fewer no-shows, fewer rehospitalizations, and a path to value-based incentives. Here's how to enroll them and use them.

Why this program

Why Prepared Provider exists

Most clinicians want to teach patients well and run out of minutes. Prepared Provider gives you the registration workflow, the engagement scripts, and the incentive map (CCM, RPM, AWV, ACO shared savings, value-based contracts) — plus the same plain-language and clinical-depth materials your patients see, so the team speaks one vocabulary.

What you'll get

  • A 60-minute orientation: how the Patient + Care Partner programs work, what they include, and how to refer in 30 seconds
  • Enrollment scripts: 'add the Prepared Patient program' in clinic, ED, discharge, and AWV
  • An incentive map: which CMS codes (CCM, RPM, AWV, BHI) the program supports, and how shared-savings programs benefit
  • PHIT dashboard hooks so you can see your panel's pre/post outcomes
  • CNE/CME-track materials when accredited

The pretest is the first step — it captures where you are before any course content, so we can show you what changes. Once it's done, you can come back to this page and pick whichever of the other cards fits your day.

What happens next

  • If something is wrong, the contact ladder is built into every course page.
