Inflection 1

Clinic — new diagnosis or new med

Clinician · 15 seconds "Now that we've named this and started a plan, there's a free Force for Health program built around exactly this — a short course in plain language, a one-page action plan, and a card with the warning signs. There's also a track for one family member or friend if you have someone helping you. Want me to send you to it?"
MA / RN · 15 seconds If yes: open FFH-Prepared-Program-Onboard-Patient.html on the room monitor, walk them through the first screen, hand or text them the URL, scan the in-room QR. Document enrollment using the practice's note macro. Tag the chart for the CCM / PCM / RPM workflow if eligible.

Between visits — care-coordination touchpoint

RN / Care manager · 30 seconds "Quick check on the home plan — were you able to start the program we sent you to? It's the one with the action plan and the warning signs. If not, I can re-send it now. If you have a family member helping you, there's a Care Partner version they can do alongside you — same depth, tuned to their job."

ED / Discharge — the highest-leverage moment

Discharging clinician · 30 seconds "Before you go home, this is your Force Field Fact Sheet for your condition — keep it on the fridge. The two red rules and four red flags on this sheet are the things that should bring you back to us. There's a full short course at the URL on the back — the post-discharge call-back form your TCM workflow uses already points there. The same site has a track for one family member."
TCM 7-day call · 60 seconds "I'm calling for your 7-day check after your hospital stay. Have you read your Force Field Fact Sheet? Can you tell me the two red rules and the four red flags? Any of those happening today? Any new medications confusing? Any equipment problems? Who is your Care Partner — and would they like the parallel program?"
