What You Do Today Can Prevent a Disease Decades From Now

A country internist started putting pins in a map and found a rare lung tumour clustered around one factory. Why the exposure question at your next appointment is not small talk.

This is the dose that gave the series its defining line. It opens with a doctor Dr. Rob never met — Richard Mann, a Mayo-trained internist in Lancaster, Pennsylvania, who died shortly before Dr. Rob arrived, and who had already worked out something important. Every quote below plays the exact moment from the recording — just tap it.

Pins on a map

Mesothelioma got one paragraph in Dr. Mann's textbook. He had never really seen it. Then he had ten, twelve, fifteen cases — tumours on the lining of the lung — and he started marking where each patient lived and where each one worked. The pins clustered: downwind of the Raybestos plant in Manheim, at the plant itself, and among people whose only exposure was washing a family member's work clothes. Others took the question up and studied it properly, and the association held.

Your work history is clinical data

Asbestos went into shipbuilding, insulation, and fireproofing through the twenties, thirties, forties and fifties. Lancaster County sent people to the Philadelphia shipyards. Decades later they came to Dr. Mann's office.

And so when you go to a doctor and they ask, "Where did you work?" They're not just making small talk.

He widens it out: radiation, heavy sand and silica from sandblasting, and the inhalation exposures he worked on after 9/11 — all of which produce disease on a clock measured in decades, on top of habits like smoking.

The line

Prevention frequently means protecting a person today from a disease that your future self can't see.

Which makes the closing ask concrete and domestic: think about it while you are raising children and grandchildren, while you choose how you paint the house, while you refinish that piece of furniture.
