Sometimes a Diagnosis Doesn't Reduce Healthcare. It Makes Healthcare Smarter.

Some of these children got a genetic diagnosis and their healthcare costs went up. Dr. Rob explains why that is the right answer, not a failure.

A new JAMA Network Open study put whole-genome sequencing in front of children with severe illness — and utilization went up. Dr. Rob takes two minutes to explain why that is exactly what should have happened. Every quote below plays the exact moment from the recording — just tap it.

What the study actually found

Dr. Rob opens on a September 17, 2026 JAMA Network Open study that followed children with severe illnesses through whole-genome sequencing. A genetic diagnosis changed the picture in two directions at once: some children turned out to be treated for the wrong thing, and the correct diagnosis surfaced additional conditions that needed managing. Care was redirected, specialist involvement was integrated, and overall utilization stayed intensive.

That is the part that trips people up. If the point of value-based care is to spend less, a study where the diagnosis leads to more testing looks like a loss. Dr. Rob says the premise is wrong.

There's a great distinction between reducing healthcare and reducing waste.

The standard he wants instead

Appropriate care and wasteful care are not the same line item, and treating them as one is how cost-cutting turns into care-cutting. He puts the alternative plainly.

Appropriate healthcare use is not something we should automatically try to eliminate.

Value-based healthcare shouldn't mean less healthcare. It should mean the right healthcare for the right person at the right time.

Why this lands on your own calendar

The same logic runs straight into screening. Knowing earlier and knowing correctly is what reduces the long tail — what he calls morbidity, mortality, suffering and cost. Which is his closing ask: if you are at risk for something, get screened, whether that is a mammogram, a colonoscopy, or a newborn screening panel that sets a child's treatment path from the start.
