What the general survey really is

The general survey is the whole-person snapshot you take with your eyes and ears in the first seconds of any encounter โ€” before a single tool touches the patient. It is where deterioration shows up first, often minutes before a vital sign ever crosses a threshold.

Think of a lifeguard scanning the pool. They are not staring at one swimmer โ€” they are reading the whole surface for the one thing that looks wrong. Your golden minute is the same scan, run on one patient.

The scan, head to toe

  • Mind โ€” alert? oriented? any new confusion or drowsiness?
  • Breathing โ€” rate, effort, posture, and how many words per breath.
  • Skin & color โ€” pink and warm, or pale, gray, bluish, clammy?
  • Strength & movement โ€” moving all limbs, or favoring one side?
  • Face & speech โ€” symmetric face, clear speech?
  • Gait & safety โ€” steady? lines, drips, and call light in reach?
The ABCs live inside the survey. Airway and Breathing come first, then Circulation (color, pulse, mentation). When two things look off at once, the ABCs break the tie.

Why the first look matters

A patient who is a little drowsy, breathing a touch fast, and speaking in short phrases may look "almost fine" โ€” and be the earliest picture of trouble. The general survey turns that vague sense of off into a reason to get vitals and compare to baseline now, not in an hour.
