You already lived this with Mrs. Carter. Now let's understand it deeply โ€” how skin breaks down, how you prevent it with your hands, and how you pass it on. Learn it, live it, share it.

Hydration keeps the whole system running

Fluid keeps blood pressure, circulation, and the kidneys working, so dehydration is more than a dry mouth โ€” it can drop blood pressure, strain the heart, concentrate the urine, and cloud the mind. A dry mouth with dark, scant urine is a signal to assess, not just soothe.

Think of a car running low on coolant. It looks fine for a while, then the engine overheats. Low fluid stresses the body's systems the same quiet way.

The dehydration cluster

  • Dry mucous membranes and skin turgor (skin tents) โ€” prompts to investigate, not proof; unreliable in older adults, whose skin tents even when well hydrated.
  • Dark, low-volume urine.
  • Fast heart rate, low blood pressure, dizziness โ€” useful signs.
  • New confusion โ€” especially in older adults.
Weigh, don't guess. A daily weight is the more reliable bedside measure of fluid change (a sudden drop signals fluid loss), and dehydration is confirmed biochemically with labs. Don't diagnose it on physical signs alone.
Elders are high-risk. Thirst fades with age and reserves are smaller, so an older person may show dehydration as confusion or a fall rather than by asking for a drink. Fever, vomiting/diarrhea, and diuretics add to the risk.

Fluid balance: I&O

Intake and output is the running ledger that tells you whether fluid in matches fluid out โ€” a core part of assessing hydration, best paired with a daily weight.
