C. difficile: the spore-former to respect

C. difficile is a spore-forming bacterium that overgrows when antibiotics wipe out the normal gut flora, causing watery diarrhea and, in severe cases, dangerous colitis. Recent antibiotics plus new diarrhea should always raise the suspicion.

Think of pulling all the weeds from a garden. Clear the healthy plants (normal gut bacteria) and the one tough weed left behind (C. diff) takes over the whole bed.

Two features make it special

  • Its spores are not reliably killed by alcohol hand rub — so soap and water is preferred (and required when hands are visibly soiled).
  • Its spores survive on surfaces — so bleach/sporicidal cleaning and dedicated equipment are needed.
  • Gloves for care, with careful glove/PPE removal, keep spores off your hands.
Act on suspicion, not confirmation. Place contact precautions and a private room, notify, and send a specimen as ordered — waiting for the test lets spores spread.

Protect the skin too

Frequent watery stool breaks skin down fast. Pair infection control with clean, dry, protect — prompt cleaning and a barrier cream — while keeping precautions and dignity.
