  8. 8. Become a CHF Disease Advocate (beyond yourself) · At your pace
Learning objectives:
  1. Weigh yourself daily and log it, and state the weight gain or symptoms that mean call the clinic today.
  2. Read and explain your own medicine/treatment list — what each is for, the dose, and the timing — and demonstrate any device you use.
  3. Sign in to your patient portal unaided, find your latest results, and bring your log and questions to your next visit.
  4. Say your personal action plan out loud, including your call-the-clinic vs call-911 red flags.
  5. Demonstrate you can take your own pulse.
  6. Call your clinic for new or worsening shortness of breath, trouble breathing when lying flat, or new swelling — even if your weight has not changed.
