Gastroesophageal Reflux Disease (GERD) is when stomach acid (and sometimes bile) regularly flows backward into the esophagus. About 1 in 5 US adults has GERD. Mild symptoms (heartburn, regurgitation) are very treatable. Untreated chronic GERD causes esophagitis, strictures, Barrett's esophagus (precancerous), and rarely esophageal adenocarcinoma. The good news: stepwise lifestyle + medication therapy controls symptoms in nearly everyone — with surgery (Nissen, LINX) reserved for the small fraction who don't respond.
About 10-15% of long-standing GERD patients develop Barrett's esophagus — a metaplastic change in the lower esophagus that raises esophageal adenocarcinoma risk. Caught early, Barrett's is treatable; caught late, esophageal cancer is one of the deadliest. Surveillance + endoscopic ablation has dramatically changed outcomes.
