Prepared Visit Tool

Walk in prepared. Walk out clear on next steps.

A three-step companion for any visit — before, during, and after. Tell your story tighter, ask the questions you came to ask, leave with a plan you actually understand.
This tool is about sharing with your clinician for your visit. For sharing outward — teaching peers, mentoring new patients, joining research, advocating — that’s the Share It chapter of your Prepared Patient Course. Two different skills, both yours.
Step 1 of 3 · Before this visit

Tell your story so your team can act on it.

Fill these in the day before (or in the waiting room). The Brief at the bottom auto-builds from what you enter — print it, screenshot it, or paste it into your portal message before the visit.

Why am I here today? (Chief Concern)

One short sentence. “I’m here for my routine hematology follow-up.” · “I’ve had a new headache pattern.” · “My pain crises are happening more often.”

Since my last visit… (the four-beat update)

Same structure every time: headline · new things · confounders ruled out · what you want them to know. Your doctor processes this 5× faster than “I haven’t been feeling great.”

Any new symptom? Describe it with the six questions.

What is it · How bad · When did it start · What made it worse · What helped · What else was new. The thing you almost forget to mention is usually the thing that changes the diagnosis.

Confounders ruled out

Check anything that’s changed since last visit. Things you check OFF are just as informative as things you check on — they help the doctor narrow down what’s causing what.

Questions I want to ask today

Tap to add the suggested ones, or write your own. They print on your brief and travel into the “During” tab as a checklist you can tap off as you ask each one.

Suggested questions for you — tap any to add it to your list.

Before I leave, I want the doctor to explain…

Write up to three. In the “After” tab you’ll teach these back in your own words — that’s how you check you actually understood.

    Your pre-visit brief

Auto-built from what you entered above. Print it, paste it into your patient portal message, or hand it to the receptionist when you check in.

Step 2 of 3 · During this visit

    Ask · clarify · capture.

Open this on your phone in the room. Tap each question off as you ask it. If anything is confusing, use the gold button. Use the bottom box to capture what got decided.

Questions to ask — tap as you ask them

From your “Before” tab. Strike-through means asked. Anything still unchecked at the end — ask before you leave.

      What did we decide?

Capture the actual decisions in plain words while they’re fresh. You’ll teach these back in the next tab.

Step 3 of 3 · After this visit

Say it back in your own words.

Teach-back is the single highest-evidence health-literacy intervention. If you can’t say it back, you don’t understand it yet — and that’s a message-the-portal moment, not a wait-six-months moment.

In my own words, what changed today is…

Don’t copy what the doctor said. Say it like you’d explain it to your sister.

What I’m supposed to do between now and my next visit

When would I act before the next visit?

Four doors. Know which one you’d use for each thing the doctor told you to watch for.

You’re done. Save your visit summary.

Saves a copy to your portal (in production), prints a clean version for your records, or copies to clipboard so you can paste it into your Health Passport / Course tracker.
