Five priorities. One living picture. Built for the people doing the work.
Maricopa County's 2024–2026 Community Health Improvement Plan names five priorities chosen by more than 18,000 residents. This Score Card puts each one on a single panel: the number, how it compares (Arizona · U.S. · Healthy People 2030), where the county ranks, which way it is moving, where the number came from, what to click next — and a coach who says what to do about it. The CHIP goal tracker sits on the same panel as the outcome it is meant to move, so partners can see whether the strategy is bending the curve.

What changed between their dashboard and this one
The mySidewalk CHIP dashboard shows Maricopa next to Arizona and stops. Every panel above adds the five things a Health Director actually needs to act — and one thing a resident needs to care.
A benchmark, not a pairArizona and the U.S. and the Healthy People 2030 target on every headline, so "is 32.1 good?" has an answer.
A rankWhere Maricopa sits among Arizona's 15 counties and 3,100+ U.S. counties, computed live from the PHIT feed.
A directionTrend lines with a target line. Overdose deaths are down four years running; homelessness set a record. Both are invisible on a two-number tile.
No "No data"Their overdose tile reads "No data" for Maricopa. MCDPH publishes 1,500 deaths for 2024. PHIT wires the county's own source.
Goal + outcome on one panelThe CHIP objective, its process indicators and the number it is supposed to move — together, so accountability is visible.
