Special Communities Snapshot
PHIT Scorecard

The soul and footprint of every community — one snapshot, scored across all 16 Force for Health Channel Communities. The front door above every individual Channel Scorecard™: see where a community stands at a glance, then drill into any Channel for the full six-lens data, tools, resources, content and games. Built for the people who live there, the leaders who serve them, and the funders who back them.

How we score each Channel

Same method on every Channel and every community — so any two squares, and any two counties, are directly comparable. Here's exactly what the numbers and colors mean.

The Channel PHIT Score (0–100)

Each Channel's score rolls up from the six standard lenses every Channel Scorecard™ shares:
HealthDemographicsDisparitiesWorkforceEconomicResources
  1. Pull each lens's underlying measures from the PHIT Data Hub™ for the selected geography (State, County, or Country).
  2. Normalize every measure to a 0–100 scale and orient it so higher always means healthier (a low ER-visit rate scores high).
  3. Benchmark against state and national figures — the score reflects how this community compares, not just its raw number.
  4. Weight toward the factors that matter most to that Channel's people, then average into one Channel Score.
  5. The square shows the top 4 headline factors; the full Channel Scorecard shows all of them across all six lenses.
How to read a square: the colored band across the top = the score tier (green Thriving · amber Building · red At-Risk) so you can scan the whole grid for where help is needed. The small Lens chip tags the channel's primary PHIT Lens (Physical / Mental / Social / Environmental). The ▲ ▼ — next to each factor shows the trend vs. benchmark.

16 Channel Communities — Top 4 Factors Each

Every Channel scores the factors that matter most to its people. Tap any square for that Channel's full PHIT Scorecard — data, tools, resources, content & games.

This is the foundation of your regional Chamber of Health®

Each community gets its own PHIT Systems instance — measuring health across every Channel, then activating prevention education, resources, tools and games where the score says it's needed most.

About the model: Each Channel Community Score (0–100) blends its four highest-leverage factors, normalized against state and national benchmarks and tagged to a PHIT Lens (Physical / Mental / Social / Environmental). Factors draw from the same 8 PHIT data-source categories that power the full Scorecard (CDC PLACES, HRSA, BLS, FDIC, EPA EJScreen, IMLS, Census/ACS, and global benchmarks). All numbers shown here are placeholder dummy data for concept review; the production build reads live values from the PHIT Datahub per State, County, or selected Country. © Force for Health® · PHIT Systems™ · Chamber of Health®.
